How do I Apply?
1. Check with your HR Dept to see if your company has a Corporate Matching program.
2. If they do, apply for the corporate match. This can be done by either filling out a paper or online form, both available through your HR dept.
3. When filling out the application, please enter Smithtown High School West PTSA as the organization name. For paper forms - complete the Employee section.
4. Return the completed form to the PTSA mail box in the school office.
